How can you give your associate a daily production sheet?

Does anyone give their associates day sheets (daily production report) using Open Dental for each day? Daily production report has patient name and procedure. This would be PHI and they would not be able to take home. Thank you

Does anyone have a dental history web form?

Does anyone have web forms other than Patient Registration and Medical History? I need electronic forms to use in kiosk for HIPAA, Financial Agreement and Dental History. Where can I find them? Thank you
