Hey Jesse,
Open Dental has webforms that may be useful for your practice: https://opendental.com/site/webforms.html
It's been a while since I've used them but the feature is solid and has been around a long time.

Jason, I think doc is asking if when expanding the screen, everything will show.
Is your screen resolution turned down? Is something missing, or is there a lot of dead space?
Increasing the screen resolution will make it so you can view a lot more. If you've got dead space, that's by design.

Instead of deleting the substitution code (that affects all plans), you can simply mark 'Do Not Substitute Codes' in the insurance plan setup window for those plans that don't downgrade.

As I mentioned before, if the work was Tx planned before the change, you'll need to adjust manually. Try charting another procedure, and it should pull the Assurant/DHA fees for you now.

You probably have your insurance plan set to Category Percentage instead of PPO Percentage.
Go ahead and change the plan type, and then update fees in the Treatment Plan Module before completing the work. If you complete work with the incorrect fee, you will have to manually edit it at that point.
