Report for grouping procedure types

Hello everyone,

I need to create a report for our bank that breaks down the percentages of each procedure type we do each year. Ex: what percentage of last years business came from hygiene, crowns, fillings, endo, etc. Additionally, I need to find a report that shows what percentage of our practice is fee for service vs. PPO. Can someone point me to a query or report to help?

UPDATE: I've figured out the procedure breakdown, but I still need to find out what our %fee for service is vs. PPO.
