Hi,
I live in Bosnia (Europe) and I'd like to translate Open Dental software and adapt it for local needs (local dentist community)
As I understood, only way to do so is to fork software (since some major changes are needed in modules like accounting)
Does anybody has experience getting CDT License (

What about plugin architecture, can it be used for greater modifications in order to avoid forking?
Any advice for localisation and adaptation is welcomed

DO NOT FORK.
We have gone to significant effort to give you a program that you can adapt without forking. To translate, see
http://www.opendental.com/manual/languages.html
You can make any change at all by using plug-ins. This will allow your users to have access to improvements in the future. It keeps the code base stable for you and for everyone.
There is no reason at all why you should need to be thinking about the CDT license. That only applies here in the US.
