Check Boxes and Static Line on Routing Slip

How can I add check boxes and a static line by each box at the bottom of the routing slip? I click "Check box" and it is not allowed???????//

Try adding a rectangle instead of a checkbox. A checkbox would only be for on-screen use, and a routing slip is always just printed.
