How are dental hygienists entered as providers?

How does this work?

Since I assume the billing is done by the dentist how does one set up the hygienist?

1. In OD

and

2. For insurances and clearing houses?

Just mark the secondary provider checkbox in their provider edit window. The hygienist can be the treating provider for provider for procedures and for the claim, but the dentist will be the billing provider for the claim.
