Insurance clarification again

Sorry, me again on the insurance thing

Could somone explain to me again when a plan would be set up as Category % and when one would be set up as PPO % and the difference in both. Thanks again

If you have signed a contract with the carrier, then it's PPO. Otherwise, it's category %.
