Is there a way to use the audit trail or another way to find out when a patients address was changed in the patient information?

I don' think there is a way to do that. There is a feature request for audit trail for family and image modules, "Audit trail for family and image modules". You can vote for that.

I didn't want to duplicate a topic for another audit trail question.
Is there an audit trail in place for procedure fee changes? I can't find who changed some of our UCR fees.

Is this related? http://70.90.133.65/forum/viewtopic.php?f=3&t=1678. It does looks like what happened. I'm running version 6.1

Yes, it's related to that link you posted. It looks like a fix was never posted for version 6.1, but was instead only posted for versions 6.2, 6.3, and 6.4. Can you just upgrade to 6.2? Very sorry about the hassle.
