Want to add outside provider to Perio Chart

Is there a way to add an outside provider to the Perio Chart? Here is the scenario: a patient comes in with their Perio Chart already completed and they want to use that. So I manually enter in the data but I want to put in the referral provider, not me. If there is an easy way to do it that would be wonderful. I am using Open Dental 5.9.
