This is used for traditional percentage insurance plans.
This was taken out of the manual directly. Could someone explain this in more detail. I am assuming this is the fee schedule used when an office does not participate with the carrier, charges their usual fee, the Insurance company pays X% and the patient pays the balance up to the usual office fee, correct?
