Can you tell me how to create a claim form? I have scanned the UB04 form and followed the procedures outlined to get the contrast higher and the specks removed but what do I do from there? I went to setup/claim forms in OD then add but the picture does not show up. I also created a folder on my local machine called opendentaldata but something is missing, the image does not show up on the page.

Is there a tool one would use to make a package that includes the image and the xml code? Then import it from there?

For starters, you have to know where your AtoZ folder is. Sometimes we refer to it as the opendentaldata folder or the OpenDentImages folder. But they all refer to the same thing:

Manually drop your new image into that folder.
