I forgot, how can I make a note about a tooth e.g crack seen on distal marginal ridge? Thanks

Sorry, I was moving to a new house that doesn't have internet yet. Anyway... There should be a procedure code for tooth note. Or you could draw it with your mouse. Or you could put it in the big box below the tooth chart.
