I have just recently updated to 6.5.22 and my custom reports button is not missing. Any ideas how to get it back? The file and folders are still intact and in the same place.

You mean the menu item, right? Not the button. In your data paths window, are you using AtoZ folder? And the name of your folder is "Reports"?
