I can see a questionnaire filled by a patient in the commlog section of the account module, but I can't find it in the chart module. I have checked/unchecked commlog and family in the "show" tab, but nothing changes. Any suggestion?

Just to clarify, "regular" comm log entries appear in the account and chart module, but questionnaires only show up in the account module: is this how it is supposed to be? This happens for me with both 5.8.8 (in production) and 6.5.5 (in test).

Questionnaires at this time only show in the comm log in the Account Module. I do not see it as a request to be shown anywhere else so you could add this to the request list in your program under Help > Request Features

That feature will soon be phased out, so it wouldn't make sense to enhance it.

What will replace questionnaire? We use it with a tablet with our new patients.
