progress notes layout

Can someone tell me where is the progress notes form as i want to make some changes to its layout.

That'd be FormProcEdit in project OpenDental > Forms

I think he means ContrChart.cs in project OpenDental, Main Modules. Or if you mean columns in the progress notes, then you should look at the already existing Display Fields feature. It doesn't require any programming; it's just a user configuration.
