It was very nice to learn that "Patient Information" form can be filled and downloaded directly into OD through Acrobat.com.
The sample of that form that was provided on OD website was great.
(Easy process to download in OD. Nice)

Do you have also a sample for the Patient Medical History Form ?
(it is always EASIER to start with a sample and modify it as needed.)
