Query for patient copay

I need a query that will return the following for a time period and specific insurance plan.

Patient name,
date of service and
copay actually paid for a specific carrier such as MetLife.

I can get the insurance payments but not the patient payment(s).

Can't seem to come up with it in the 605 examples listed
Any help would be greatly appreciated.

Upon further review and with fresh eyes this AM I was able to modify #93 slightly to give we what I wanted.
If anyone needs the modification for a specific carrier, let me know and I will post it here.
