Here is my current medical history form, modified from the one provided in a recent release.
This is not quite as thorough as I have planned but I think it is much more thorough than the "stock" medical history form.
Utilizes the medication, allergy and problem importation features of the recent release.

This is of course from an OMS perspective, and will likely be most useful to any other oral surgeons that are considering using OD.

are you able to post a screen shot of the form?

8 years later, here is a screenshot of the form. I have been using a more thorough paper form, but with the coronavirus, I am re-exploring electronic check in procedures.

How do you handle 'updates'?
A different abbreviated update type sheet, or same thorough one over again?

The thread is about an expanded electronic medical history form for dental/oral surgery use. It includes medications, allergies, medical conditions, patient details, visit information, and a signature section. A later reply asks how to handle updates to the form.
