can you set the appointment view to see for example: age, insurance plan ? i mean not as a note,as a default

Yes, use the Age and HasIns from the list of Available Rows.

But to see which plan they have, Aetna, MetLife, Delta, you have to setup a custom field, select where you want it to show and manually enter the name in that field.
