I want to employ a programmer to create an insurance claim form in OD specific to Ireland where I practice. I am unable to do this through the 'Claim Form' facility in OD because the form is in landscape and for other reasons. I intend to employ a programmer to do this work for me. I want to create a dummy patient database with all the functionality of my OD software to give to the programmer to work on in order to protect my patient data. Can anybody advise how I do this.

You should have him simply install the database from the trial edition. Then you can set up one test patient / carrier / provider / insurance so he'll have a test case to know when your requirements are met. I don't think it would take that long at all.

I cannot get the demo version to work like my own practice version. In particular, when I export the fee schedules and the fee codes, the codes do not export and the fees are not present either. This info is necessary for the completion of the insurance claim form.

I can't see why it's necessary. I program such things every day and I use dummy codes and made up fee schedules. Once you use it with your live database, at that point the claim will populate with the valid values.
However, if you really need to, you can use the procedure code export tool (Please note that it would be a violation of the copyright to share the ADA procedure codes with another office) to export the codes themselves: http://opendental.com/manual/procedurecodes.html
and then you would export your fee schedules. Then on the trial side, import the proc codes then import the fee schedules.
I still feel you do not need to do this for the programmer.
