eServices sending some emails but not all?

We've just started using the eConfirmation and eReminder services and are trying to work out some kinks.
It appears that some patients are getting the email / text and some are not. We noticed that on one of the upcoming days, one patient got the email / text and another unrelated patient did not. Yet, their settings / preferences are listed as the same.
Any idea why this happened?
