Report for ADA codes per clinic per provider

Hello,
I was wondering how to adapt the current report to separate income generated by certain codes (or code groups) per clinic? We use a "Clinic" feature and want to see the report on one clinic separate from the other one.
The codes we are looking for on the report are:
a. How many D1110, D1120
b. How many D0150
c. How many D0120
d. How many D0140
e. How many D2740
If the codes are set up as a range, I can modify sql query myself, but for some reason I could not get a desired result with the existing query.
I'm using a query #1135. Can you suggest how to format the query to limit the patients assigned to only one clinic? That would be all I need I think.
