Description field in statement

Hi there,
I have a problem in statement page. I get Provider's first name repeated three times in description field.
I have uploaded the image of the statement to google drive for your reference mentioned below. Provider's first name here is "Sumeet" in the uploaded image.

What is wrong with my configuration?

One of our engineers committed a fix for this problem in versions 21.1.53 and 21.2.27. What version are you on?

I did upgrade the OpenDental to 22.2.36 and the issue was resolved. thank you for your response.
